New Client / Patient Form

New to LABAH? Fill this out before you come to speed up your initial admission. Please bring copies of all available medical records, or have them faxed to us in advance to make the most of your initial appointment.

Drop Off Form

Already a client and you have scheduled a drop off? We need to know what for. This means you can deliver your 4-legged bundle of joy and still make it to work on time without being delayed by all the paperwork.

Anesthesia Authorization Form

If you are leaving your pet with us for a procedure that requires an anesthetic you will be asked to complete this form. Save time, print, complete and bring it with you.

Client Information Change Form

Did you move? Get a new cell number? Make sure you let us know when you come in so we can keep your record up to date. Better still, fill this in and bring it with you when you come for your next appointment.

Grooming Form

We will ask that you fill this in when you drop off your pet with the groomers. Speed things up by filling this in at home before you come in.
